For ALL returning FALL or SPRING members please make sure you have downloaded the Medical form and Parent Consent form. The medical form must be filled out and stamped by your child's physician. At the end of the form you will be asked to upload the Medical Form and The Parent Consent Form. All forms can be downloaded here: https://prospect-park-youth-running-club.myshopify.com/pages/forms . 

For ALL returning WINTER or SPRING members you do not need to submit Medical form or Parent consent. 

Once you have submitted the registration form (see below) with all required documents, please proceed to the Registration Payments page to complete your registration. *YOUR REGISTRATION WILL NOT BE COMPLETE WITHOUT PROCEEDING TO THE PAYMENT PAGE.
